Plot (credited to AsianWiki):

Kim Sabu and his team at Doldam Hospital continue their work to save the lives of patients. The old Doldam Hospital building is set to welcome a new connecting trauma center building that features the latest in medical equipment. Prior to the trauma center's official opening, a crisis occurs on a North Korean ship that has drifted into South Korean waters. Doctors from Doldam Hospital fly out to help the patients on the ship who are in critical condition, but the patients must be brought to the hospital. Meanwhile, the South Korean government wants the North Korean patients returned to their country in order to have a smooth summit with its neighboring country. A surprise visitor also awaits at Doldam Hospital.

Cha Jin-Man is brought to Doldam Hospital by Park Min-Guck to run their new trauma center. Cha Jin-Man is the father of Cha Eun-Jae and an old rival Kim Sabu.

Three years after Doldam Hospital became a separate entity from Geosan Foundation which put the end of Chairman Do's ability to sabotage the hospital and Kim Sabu, the trauma center that was once a dream, almost became a reality. With Park Min Guk as the hospital director, replacing the late President Yeo, the trauma center almost reaching its successful beginning. However, it will not be Dr. Romantic without unexpected surprises and turn of events. Starting with a vengeful mother slash assemblywoman who is responsible for overseeing the province government budget approval, a troublesome resident doctor, a new trauma center with different values and views from Kim Sabu, and natural disasters, it was full of dramas and action. Not to mention, the variety of patients and love among the doctors and nurses.

How does this drama franchise keep getting better from one season to another because hello?! We got so much medical action and drama but balanced with the romance side of it. After years of following this series, safe to mention I have, without shame, rewatched it more than 5 times. The scenes were not only restricted inside the hospital only but there were scenes at a collapsed building and inside a freaking ship.

I love the new character addition. The newbies doctors; Dr. Dong (Jang Dong Hwa) and Dr. Ung (Lee Seon Ung) were both cute in their own way. And if I am not mistaken, Dr. Ung was the evil spirit in the first season of The Uncanny Counter. I can't believe I recognized him lol. I personally love the connection between Dr. Dong and Kim Sabu. It somewhat feels like a closure for Kim Sabu for the loss of his first pupil.

Compared to the other season, this season had a large cast, especially with the additions and Kang Dong Ju coming back, although he only came like for the last 4 episodes. However, each character had their own personality and quirked that add charms to this drama. I personally love the interaction between Director Park and Nurse Oh. It was so cute to see them. And when Dr. Dong asked who is Dr. Areum's boyfriend to the boyfriend's face (Nurse Eun Tak), to which he respond, "Like this. Dr. Areum's boyfriend look like this". Comedy goals. I love it.

I really hope to see one last season of this drama with all the cast from the first season; Kang Dong Joo, Yoon Seo Joong, Do In Beom, Woo Yeon Hwa, and the current cast of season two and three together. And of course, we have to see Kim Sabu's love life come to conclusion.
